Document checklist

Ask Admissions to confirm the current requirements for your child’s grade and circumstances.

Online registration form
Complete the school’s Google Form using the pre-registration page.
PSA birth certificate
Prepare a photocopy.
Form 138 or report card
Ask Admissions which school records are needed for your child’s entry level.
Medical certificate
The published list specifies a photocopy for face-to-face learning. Confirm whether it applies.
Long white folder
Prepare a folder with a fastener.
Two 2 × 2 ID photos
Confirm the current photo specifications with Admissions.
